The documents the model asks you to produce
Three free outlines, each with the clause reference beside every section. The paid kit adds the fill-in workbook with the owner, status, due and evidence columns. The outlines are ours, written from the model; they are not the standard's own forms.
AI quality requirement sheet
The buyer's fill-in document. One row per characteristic in scope, with the task, the measure, the method, the threshold, the conditions, the owner and the evidence. It is the document the kit and the record are built for.
AI quality evaluation plan
The plan that turns the requirement sheet into an evaluation. It names the method, the data, the people, the acceptance rule and the record for each characteristic.
AI service quality annex
The annex for an AI system delivered as a service. It adds the three service characteristics to the requirement set and states the service boundary, the support and the exit.
How the three fit together
The requirement sheet is the decision: what the system must meet, on which characteristic, with which measure. The evaluation plan is the test: how each requirement is evaluated, on what data, by whom, against what rule. The service annex is the addition for a system delivered as a service. A governance lead who has all three can answer a board, a customer and an assessor with the same documents.
